CBD for senior dogs with arthritis — what the Cornell study actually showed

Roughly one in four SA dogs over the age of seven has clinically measurable osteoarthritis. The owners who notice it most are the ones whose dogs used to bound up the stairs and now hesitate. CBD comes up early in every senior-dog conversation, often before the vet has even diagnosed the cause. This page sorts the hype from the evidence.

Talk to your vet first. CBD is not a substitute for diagnosis and staging. The point of an arthritis work-up is also to rule out other causes (hip dysplasia progression, neurological pain, soft-tissue injury) where CBD does much less.

What the Cornell study showed

The 2018 Gamble et al. study at Cornell University’s College of Veterinary Medicine remains the strongest published evidence for CBD in dogs. 16 dogs with confirmed osteoarthritis, 2 mg/kg of CBD oil twice daily for 4 weeks. 80% showed measurable improvement in pain and mobility scores on validated owner-reported and vet-assessed metrics. No clinically significant adverse events; mild elevation of liver enzymes in some dogs that resolved off-CBD.

This is what made “2 mg/kg BID” the de-facto starting dose for canine arthritis — it’s the dose that worked in the only study designed to test it.

What CBD actually does for arthritic dogs

CBD is an anti-inflammatory, not a direct analgesic. It modulates the inflammatory cascade in joints (via CB2 receptors and other pathways) rather than blocking pain signals the way an NSAID does. The practical consequence:

• Onset is gradual. Day 1 doesn’t look like anything. Week 2 you may see less stiffness in the morning. Week 4 is the assessment point.
• Best fit: chronic, low-to-moderate joint pain with persistent inflammation.
• Poorer fit: acute flare-ups (NSAIDs are faster), neurological pain (CBD has limited effect), end-stage hip dysplasia (surgery / advanced analgesia territory).

The senior-dog stack that actually works

CBD is one lever, not the lever. The honest order of impact for most SA senior dogs:

1. Vet diagnosis. X-rays, joint exam, possibly advanced imaging. Confirm osteoarthritis vs. dysplasia vs. cruciate vs. soft tissue.
2. Weight management. Every excess kilogram on an arthritic dog is amplified at the joint. The single biggest non-drug intervention.
3. Joint supplements — glucosamine, chondroitin, MSM, omega-3 (EPA / DHA). Slow build over months.
4. Controlled exercise — consistent low-impact walks, swimming if available, no weekend-warrior bursts. SA hydrotherapy facilities exist in most metros.
5. Prescription analgesics if needed — NSAIDs (under vet supervision; CBD interacts with these via CYP), adjuncts.
6. CBD as adjunct — never as the only intervention.

Practical dosing for SA senior dogs

A 25 kg Labrador at 2 mg/kg BID = 50 mg per dose, twice daily = 100 mg total daily. With a 600 mg/30 ml tincture (20 mg/ml = 1 mg/drop), that’s 50 drops per dose — a meaningful volume, often easier to give as a measured ml (2.5 ml from a syringe). Higher-strength tinctures (1000 mg / 2500 mg in 30 ml) make administration easier for larger dogs.

A daily routine for arthritic SA senior dogs

• Morning: CBD dose with breakfast. Short walk before the heat builds. Joint supplement.
• Midday: Cool, comfortable rest space. Avoid hot tiles — soft bed.
• Evening: Second CBD dose with dinner. Gentler walk in the cool. Possibly a brief massage of the affected joints if your dog enjoys it.
• Weekly: Honest mobility check — same route, same time, note any change.

When to call the vet

• Sudden worsening or new asymmetric lameness.
• Reluctance to eat, vomiting, persistent diarrhoea.
• Behaviour change — withdrawal, snapping, sleep disruption.
• No measurable improvement at week 4 of consistent CBD dosing.
