Medical cannabis in South Africa — the Section 21 pathway

Medical cannabis in South Africa is not over-the-counter. Schedule 4 (THC/CBD tinctures) and Schedule 6 (cannabis flower) products are dispensed only after a HPCSA-registered practitioner reviews the case and, where required, SAHPRA grants a Section 21 authorisation for patient-specific access to a medicine not yet registered for general sale.

This is the same regulatory pathway used for any unregistered medicine in SA — orphan-drug oncology agents, certain rare-disease therapeutics, and yes, medical cannabis. The legal framework is the Medicines and Related Substances Act 101 of 1965, Section 21.

How the Cannabuben + Docto24 process works

Step 1 — Browse + add to cart. You select the product on this page and pay through standard checkout. The order doesn't dispatch yet.

Step 2 — Clinical screening. Immediately after payment you complete a short post-payment questionnaire. Honest answers matter — the clinician uses this to decide if the request is appropriate for you.

Step 3 — HPCSA practitioner review. A registered SA practitioner reviews your screening, usually within 24 hours. Three outcomes: authorise, request more information, or decline. Declined orders are refunded in full (product cost only — clinical review is not separately charged).

Step 4 — SAHPRA Section 21 application (where required). For most medical cannabis products, the practitioner files a Section 21 application on your behalf. SAHPRA processes typically take 2–3 business days.

Step 5 — Dispensing pharmacy. Once authorised, our partner pharmacy dispenses the product against the SAHPRA approval and couriers it directly to you. Cannabuben does not hold or dispense Schedule 4/6 medicines — the pharmacy does.

Total time from cart to delivery: typically 3–5 business days for products requiring Section 21, 24–48 hours for already-authorised stock.

Cost structure

Listed prices include the medicine + dispensing pharmacy fee. The clinical-review fee is bundled into the product price — no surprise charges. SAHPRA application fees (when applicable) are also covered.

Section 21 medicines are generally not covered by SA medical aid schemes. Some schemes reimburse the clinical-review fee under tariff code 0130 — check with your scheme directly. We can supply a tax-invoice itemising the clinical-review portion if your scheme accepts it.

What medical cannabis is — and isn't — for

Medical cannabis under Section 21 is most commonly authorised for: chronic neuropathic pain that hasn't responded to first-line treatments, refractory insomnia in a clinical context, palliative-care symptom management, certain neurological conditions (drug-resistant epilepsy, MS spasticity), and chemotherapy-induced nausea.

It is not the right pathway for: general wellness, anxiety as a primary presentation (talk to your GP about first-line treatments first), recreational use, or any concern that hasn't been properly diagnosed by a clinician.
